Pearson is two terminals and a lot of doors, and the arrivals level looks the same in both directions. These are the spots we use every day. If you booked meet and greet you do not need any of them, because your chauffeur finds you first, but it is worth knowing where the car will be.

You ask for it when you book, and the amount is confirmed inside your one flat rate before you agree to anything. Nothing is added at the end of the ride.
